Project Access-New Haven (PA-NH) was founded in 2009 by a group of dedicated physicians who were concerned about inadequate access to medical care – and resulting health disparities – among low-income, uninsured and under-insured individuals in Greater New Haven. PA-NH addresses this need by working to increase access to medical care and services for under-served individuals in the Greater New Haven area. We achieve this mission by expanding the network of care available to under-served patients in our community and providing intensive patient navigation to remove access barriers and facilitate the timely delivery of comprehensive, coordinated care to those most in need.
